Dumb post time...
But my therapist suggested I write down a list of ways my disability/pain gets in the way of every day activities etc... my brain is... cloudy right now. Nerves, I think.
So you guys know me. You know my usual complaints. I have a few obviously but I know I could add more and there's probably stuff that's not even occurring to me :|

driving hurts.
well, you like to dance, but from what I've seen, it seems to be a bad idea sometimes :P
I know with MY back issues, doing housework can be a pain due to repetitive bending, etc.

I'm guessing she asked you to do this to set your mind at ease and organize your thoughts for the up coming VA evaluation. Which I think is a really good thing to do.

I experience varying levels of pain from day to day. Some days I can function like I used to before the pain started. Other days I can not even get out of bed.
On a good day I would rate the pain as a X (on 1-10 scale), and it affects my life X (% of 100).
On a bad day I would rate the pain as a X (on 1-10 scale), and it affects my life X (% of 100).
Ways the pain affects my life:
Limits my ability to find a job because standing, siting, or walking for extended periods of time causes pain.
There are days I am unable to get out of bed because it hurts to much to move or stand.
The pain affects my ability to do normal tasks on a daily basis; like cooking, getting the mail or going grocery shopping.
At times I can not drive or walk on my own and must use a cane, wheel chair or have someone else assist me.
I take (pain medicine) to ease the pain X times a day/week. It alleviates the pain X%.
On average:
Unable to stand for X amount of time
Unable to sit still for X amount of time
Unable to walk more than X amount of time/miles
Can not drive more than X amount of time
I can not run or jog - this significantly reduces my likelihood of surviving a zombie attack
